The online therapy industry exploded in 2020-22 and partially imploded afterwards over data-privacy issues (BetterHelp's 2023 FTC settlement, Cerebral's 2023 issues with controlled-substance prescribing). In 2026, the market is mature but cautious — there are good options and meaningfully bad ones.

The licensed-therapist platforms

  • BetterHelp — paid (~$60-90/wk); largest network of licensed therapists; 2023 FTC settlement over data sharing with Facebook / Snapchat / Pinterest; fixed but the trust hit lingers.
  • Talkspace — paid; in-network with major US insurers (Aetna, Cigna, others); often $0-30 copay; the most-likely-covered-by-insurance pick.
  • Brightside — paid; medication + therapy combined.
  • Cerebral — paid; controlled-substance issues 2023; more cautious now; medication + therapy.
  • Sondermind — paid; takes insurance.
  • Alma / Headway — therapist-finder marketplaces; in-network options; pay therapist directly.

AI-flavored / chatbot

  • Wysa — paid + free; CBT-based AI chatbot + optional human therapist; NHS-validated for some uses; one of the more-tested AI options.
  • Woebot — paid + free; CBT chatbot; scaled back in 2024.
  • Replika — paid (~$20/mo) + free; "AI friend" — not therapy; controversial; some users derive real benefit, others form unhealthy attachments.
  • Earkick — paid + free; AI mood / mental-health companion.
  • Pi (Inflection) — free; conversational AI; not licensed therapy.

Self-host / private alternatives

Privacy considerations

  • BetterHelp's 2023 FTC settlement required them to pay $7.8M and stop sharing health data for advertising. Read the privacy policy of any platform — and assume health-app data is not automatically HIPAA-covered.
  • Insurance-billed therapy is HIPAA-covered (provider side); the platforms layered on top vary.
  • AI chatbot conversations are typically not HIPAA — your messages can train future models depending on the policy.

  • Crisis: 988 or local equivalent. Not these apps.
